Health Connections: Cardiac & Pulmonary Rehabilitation

Health Connections, a service of Houston Heart Institute and Houston Healthcare, is an outpatient rehabilitation program designed to help people who have experienced heart disease or pulmonary disease improve their quality of life and return to full, productive lives. Available by physician referral, Health Connections offers a comprehensive program including individualized exercise therapy, education, and counseling for participants and their family members.

Located at Houston Medical Center in Warner Robins, Health Connections provides a convenient, comfortable environment with valet parking for patients.

Health ConnectionsUpon entering the program, each patient receives a thorough health assessment to determine his or her current fitness level and an optimum exercise program. An individualized exercise regimen is supervised by a qualified exercise physiologist, registered nurses, and a respiratory therapist using advanced EKG and Pulse Oximetry monitoring. Individuals receive education and counseling regarding nutrition, ideal body weight, smoking cessation, diabetes management, stress management, better breathing, and other lifestyle changes. During exercise, patients learn about appropriate exertion levels and the importance of regular blood pressure, target pulse, and pulse oximetry checks.

Health Connections focuses on Phase II cardiac rehab and pulmonary rehab which includes cardiovascular training, strength training, and further education about cardiac and pulmonary illnesses. The center features a 12-week, 36 session program for cardiac patients and a 8-week, 24 session program for pulmonary patients. Rehab may begin within two weeks after discharge from a hospital or healthcare facility.

Health Connections also offers a Phase III Maintenance Program to benefit those members who have already been through our previous rehabilitation programs and who would like to maintain a healthy lifestyle. The Maintenance Program is extended to those who have finished a cardiac or pulmonary program from another facility.

The program is administered by a team of nurses, respiratory therapists, exercise physiologists, dietitians and other health professionals trained in cardiac and pulmonary education.
