Order Films

To check out your medical films at Perry Hospital, you may contact the receptionist at 478.988.1649 and tell her when the exams are needed. It is best to give 24 hours notice, as the films have to be printed. Certain exams may take up to an hour to print. If you need to obtain a copy of a Radiology report, you may call the Health Information Management (Medical Records) Department at 478.988.1620.

However, you may now request a copy of your films online, but you must pick them up from the Radiology Department at Houston Medical Center.

To order your films online, fill out the spaces below and submit. Please call 24 hours before the day of pick up to ensure your films are ready.

If you are checking out your films, a picture ID must be shown in order to receive your films. If you are checking out films for another party, a picture ID of that person, as well as your own photo, ID MUST BE PRESENTED.
